There are no claims to file, no preauthorization is required nor is there any annual or lifetime maximums. Members pay the Provider's office at the time of service. Simply choose a participating Provider from the directory to receive your covered benefits. At the time of your visit, tell the Provider you are a member of DENTAmax Plus.
