The DENTAmax Plus Plan

Service Selection

To receive treatment with the DENTAmax Plus (DMP) reduced fee plan, a provider must be selected from the list of participating providers.  Any licensed provider is eligible to participate in the plan as an independent contractor subject to approval by DMP.

Enrollment

To become a member of the plan:

Complete the enrollment application, including, all household members to be covered by the plan and the method of payment selected.  If you are not a participant in a group plan, there are several methods of payment, below is a break down of each:

Monthly payments-Payroll deduction (if participating with a group), Visa/MasterCard, Bank Draft

Semi-Annual payments-Receive 1 months free membership, Check, Money Order or Bank Draft, Visa/MasterCard

Annual payments-Receive 2 months free membership, Check, Money Order or Bank Draft, Visa/MasterCard

Enclose the above along with the $15.00 enrollment fee and mail to:  DENTAmax Plus, 3012 19th Street, Suite B, Metairie, LA 70002.  Upon receipt and approval, your membership card and provider listing will be forwarded to you.

Fees for Services

Payments for Services are made by the member directly to the Provider's office.   If long term treatment is necessary, the member should discuss the treatment and payment arrangements with the Provider.  Any procedure that is not listed on the fee schedule will be charged to the member at a reduced fee of 20% of the usual and customary fees charged by the Provider.  Providers with an (*) next to their names charge a fee equal to their regular fee minus 20%.  Providers with two (**) next to their names charge a fee equal to their regular fee minus 15%.  Fees are subject to change without notice.

Limitations and Exclusions

Treatment for injuries or conditions that are covered under Worker's Compensation or Employer's Liability Laws, Automobile, Medical, No Fault or similar type insurance services which are provided without cost to the member by any county, municipality, or other political subdivision is excluded.

Any treatment already in progress will be excluded.  Special arrangements may be made at the option of participating providers to assume treatment in progress.  Fees for assumption of treatment should be negotiated by Provider and Member.  These fees may or may not be relative to the DMP member fee schedule.

Fee schedules apply only when treatment is performed by a participating Provider.   If the service of a nonparticipating Provider is required, or services are performed in a hospital facility, these fees do not apply and the member is responsible for charges incurred.

General Information

If a Member needs to cancel a scheduled appointment, the Member must notify the Provider's office 24-hours in advance to avoid a cancellation fee.

The DMP plan may not be used in conjunction with other plans or insurances.  DMP   is to be considered secondary to all other plans or insurances.

Member must identify himself as a DENTAmax Plus member at time of service to receive savings.
