Available to residents in the state of Louisiana only.

     Dentamax Plus, Optimax, Chiromax, and the Hearing Care, Dermatology, Plastic Surgery, and Prescription Drug Plans are access plans that provide substantial fee discounts to members by obtaining services from participating dentists, opticians, hearing care specialists, chiropractors and pharmacies. In addition, affiliated ophthalmologists offer refractive surgery (including Radial Keratotomy (RK) and PRK, procedures to correct nearsightedness) at a substantially reduced fee to Dentamax Plus members. Membership fee includes all services.
